Request a Course
If your cohort group or school corporation would like an elective course taught onsite, you may request one by filling out the Add-on Request Form. You must have a minimum of 8 students in order to run the class.

Instructions for the Add-on Request Form
Please review the following instructions, then complete the Graduate Electives Add-On Request Form (.pdf)*.
Due Date
This request is due eight (8) to ten (10) weeks before the projected class start date. There must be a guarantee of no fewer than eight (8) students.
Course Number
Find the course number and title on the list of IWU electives. See http://caps.indwes.edu/electives/graduate/education/ for more information.
Credits
All courses are three credits unless otherwise specified.
Prerequisites
A bachelor's degree is the prerequisite for a course unless otherwise specified in the IWU electives list. Check the course descriptions for more information.
Class Dates
List every date that the course meets. Do not just put in a range of dates.
City/Online
Fill in the complete information on the site. If you are not using an IWU Education Center, we will approve the site.
Special Circumstances
Note any special circumstances, such as the course is being taught during the entire school year, or is onsite with a Blackboard component.
Facilitator
Students may request a facilitator OR a facilitator may request a course.

Payment Information
If you are a facilitator requesting a course, fill out the payment information.
Tuition and Books Costs
Tuition and book costs for current courses can be found on the website. For other courses, if you wish textbooks or other resources for your class, you will have to advise students to purchase those on their own.
Application Fee
All students who have never taken a course from IWU must pay a $25 application fee so the answer to this question is usually "yes." However, under certain circumstsances, this may be waived and in this case, you will be notified.

Student Names
Please include the names and year-around email addresses of at least eight (8) students who are committed to taking the course. This does not register them for the course. An official registration form will be sent when the class has been added to the schedule.
Registration forms are due thee weeks before the expected start date of the class.
